Primary Care
Welcome to the Naval Health Clinic Corpus Christi Primary Care. Primary Care is the central resource for your health needs: urgent, preventive, and routine. Active duty personnel are assigned a Primary Care Manager (PCM) that will work with you through out your visit with us. Primary care Provides outpatient care relative to the examination, diagnosis, treatment and referral to consultative services of enrolled patients of all ages up to age 65.
Patients are assigned a primary care provider and a medical team based on the Patient Centered Medical Home Port model. Primary Care Is staffed with family practice, pediatric and internal medicine physician assistants and family nurse practitioners as Primary Care Managers (PCMs), who collaborate with other clinical services to provide comprehensive patient centered and team oriented family-based healthcare.
Primary care is also staffed with disease management nurse, and case management nurse(s), if required by instruction, who collaborate with primary care managers and their Medical Home Port Team members to provide comprehensive, holistic patient centered care. We are staffed with Independent Duty Corpsmen (IDCs), who provide acute care access to and coordinate outpatient acute care relative to the examination, diagnosis, treatment and referral to consultative services of non-flight status military enrollee.
